For more than a century, insulin has transformed diabetes from a fatal disease into a manageable chronic condition. Yet despite continuous advances—from glass syringes to insulin pens and pumps—one aspect has remained largely unchanged: most insulin must still be injected.

Now, that paradigm is beginning to shift.

Afrezza®, developed by MannKind Corporation, is the world’s only commercially available inhaled mealtime insulin. Following its launch in India by Cipla and a recent expansion of its U.S. Food and Drug Administration (FDA) approval to include children and adolescents aged six years and older, the therapy is drawing renewed attention as a needle-free alternative for mealtime insulin delivery.

What Is Afrezza?

Afrezza is an ultra rapid-acting human insulin delivered as a dry powder through a small handheld inhaler. Instead of being injected under the skin, insulin is inhaled into the lungs, where it is rapidly absorbed into the bloodstream through the lung’s large surface area.

The therapy is designed to control the rise in blood glucose that occurs after meals. Patients inhale the insulin at the beginning of a meal, eliminating the need for multiple daily mealtime injections.

Unlike long-acting (basal) insulin, Afrezza is not a replacement for basal insulin. People with type 1 diabetes must continue using long-acting insulin in combination with Afrezza, while some patients with type 2 diabetes may use it alongside other diabetes medications depending on their treatment plan.

How Does It Work?

Afrezza uses MannKind’s proprietary Technosphere® technology.

The insulin is packaged into microscopic particles made from fumaryl diketopiperazine (FDKP). Once inhaled, these particles dissolve rapidly in the lungs, allowing insulin to enter the bloodstream within minutes.

Because of this rapid absorption, Afrezza begins lowering blood glucose in approximately 12 minutes, more closely mimicking the body’s natural insulin response to meals than conventional injected rapid-acting insulins. Its action also ends relatively quickly, reducing prolonged insulin exposure after meals.

A New Option for Millions of People in India

India is home to one of the world’s largest populations living with diabetes. According to estimates cited by Cipla, nearly 100 million adults in the country are living with diabetes.

Recognizing this growing healthcare burden, the Central Drugs Standard Control Organisation approved Afrezza for adult patients, and Cipla has launched the product as India’s only inhaled insulin, following an exclusive licensing and distribution partnership with MannKind.

The launch offers a new treatment option for adults with both type 1 and type 2 diabetes who require mealtime insulin but may struggle with multiple daily injections.

FDA Expands Approval to Children

Afrezza recently reached another important milestone in the United States.

The U.S. FDA approved the therapy for children and adolescents aged 6 years and older, making it the first and only inhaled mealtime insulin approved for pediatric patients with diabetes.

The expanded indication was supported by the Phase 3 INHALE-1 clinical trial, which evaluated Afrezza in children and adolescents with type 1 and type 2 diabetes.

Advantages of Inhaled Insulin

For many patients, one of the biggest barriers to insulin therapy is fear of injections.

By replacing mealtime injections with inhalation, Afrezza may improve treatment acceptance and adherence in appropriate patients.

Potential advantages include:

  • Needle-free administration
  • Ultra-rapid onset of action
  • Better control of post-meal blood sugar spikes
  • Shorter duration of insulin activity, more closely resembling normal physiology
  • Portable inhaler with simple dosing cartridges

These features may be particularly valuable for patients who delay insulin initiation because of injection-related anxiety or inconvenience.

Who Should Not Use Afrezza?

Despite its convenience, Afrezza is not suitable for everyone.

Because insulin is delivered through the lungs, the therapy is contraindicated in people with chronic lung diseases such as asthma or chronic obstructive pulmonary disease (COPD) due to the risk of acute bronchospasm.

Patients must undergo a lung function test (spirometry) before starting treatment and periodic monitoring during therapy.

Afrezza is also not indicated for treating diabetic ketoacidosis and should not be used by people who smoke or have recently stopped smoking.

A Different Way of Thinking About Insulin

Afrezza represents more than just another insulin formulation—it introduces a different route of administration that has been pursued for decades but rarely achieved commercial success.

Previous inhaled insulin products struggled because of bulky delivery devices, limited patient acceptance, and commercial challenges.

Afrezza’s compact inhaler and ultra-rapid pharmacokinetic profile have helped distinguish it from earlier attempts, although long-term adoption will depend on physician confidence, patient education, reimbursement, and real-world clinical outcomes.

Looking Ahead

The introduction of inhaled insulin in India marks an important milestone in diabetes care.

For patients who require mealtime insulin but wish to avoid injections, Afrezza offers a novel therapeutic option. At the same time, its recent pediatric approval in the United States reflects growing confidence in the technology and expands access to younger patients living with diabetes.

While inhaled insulin will not replace conventional insulin therapy for every patient, it broadens the range of treatment choices and underscores a larger trend in diabetes care—developing therapies that are not only effective but also easier and more acceptable for patients to use.
